DESCRIPTION OF WORK

As an Adult Cross-System Coordinator, you will be providing follow-up and technical assistance in procuring and linking appropriate resources. Duties include offering outreach and education about the DHS program offices and higher-level of care programs that are offered through HealthChoices, as well as serving as a designee on various projects throughout the entire DHS program offices. You will have the opportunity to function as a program monitor for the behavioral health/developmental services under the supervision and direction of the Complex Case Director. In this role, you will support planning and staffing efforts to serve adult consumers identified in two or more of the following groups:

  • Being discharged from psychiatric inpatient or state hospitals
  • Admissions to Long Term Structured Residence or any other long term care facility
  • At risk for multiple inpatient readmissions
  • Seriously Mentally III (SMI) consumers experiencing reoccurring acute episodes
  • A member of a priority or special needs population (i.e. Intellectually Disabled, Aging, or Forensic) who present with complex behavioral health, developmental services, medical and/or other life care needs

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:00 PM, Monday – Friday, with a 30-minute lunch.
  • You may be required to work beyond operational business hours to attend emergency situations.
  • Telework: You may have the opportunity to work from home (telework) part-time up to 2 days a week, upon successful completion of the required probationary period. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Greensburg.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Two years of professional level experience in the field of mental health; and a bachelor’s degree; or
  • Any equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Westmoreland County. If no eligible candidates who live within Westmoreland County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.

Legal Requirements: 

  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
